Why it is necessary to treat sewage before disposing it off in a water body?

Your body is immune to the critters in your own gut, but maybe other people are not, and maybe you are not immune to theirs. You don't really want to give all the critters free rein in open water, so you try to kill some of the critters before you dump. That is good enough as long as you don't have too many people in one place.
