Where can I get fingerprints taken for Police Clearance in Madrid, Spain?

1) For foreigners in Spain, who need a police clearance back in their home countries. You need to go to Carreta General Policia Cientifica in Pinar del Rey, Metro Line 8. Download the fingerprint form from your country and take it with you to the police station with your I.D

Do you know if you need an appointment for this service? I found some info on the US Embassy page saying you needed to first get a notarial letter ($50) from the Embassy before making an appointment with the Policía Científica -- is that necessary?
