This can get complicated so I've drawn you a family tree diagram:
First you find the common ancestor. If you share a parent, you are siblings. If you share a grandparent, you are cousins. Great-grandparent = second cousins. Great-great-grandparent = third cousins.
So your grandma and her cousin have a common ancestor in a grandparent. Your grandma's grandparent is your great-great-grandparent. So you and your grandma's cousin (and the cousin's children) are third cousins (because you share a great-great grandparent).
Then you have to work out how many times "removed" you are. If you are on the same level on the family tree you are not "removed" at all. If you are one level up or down you are "once removed" and so on, and so on.
Your grandma's cousin is your third cousin twice removed and your grandma's cousin's son is your third cousin once removed. If he had any children they would just be third cousins.
It's close to the other answer, but you go with the closer term. In other words, the gentleman in question and you would share his great and your great great grandparents. People that share: grandparents are first; great grandparents are second; great great grandparents are third. Also, since you aren't equal, you will have to use "removed" to show how many different levels. He's closer, so you would be 2nd cousins and you are one different layer so you would be once removed, so you're second cousins, once removed. Your grandma's first cousin and you would be first cousins, twice removed.
