What treatment do I need to do for plantar fasciitis?

I have plantar fasciitis in both feet. I tried wearing splints at night, but after I took a few steps in the morning, the pain came back right away. Then I had orthopedic "moon boots" for 12 weeks, but the pain came back soon after I quit wearing them. They didn't immobilize my feet well so there was no healing. The solution that has had the longest effect was below the knee fiberglass casts that extended all the way under my toes. I was in these casts for a total of 8 weeks, with new casts at 4 weeks. The casts relieved my pain for over 2 years. When it flared up again, I wore casts again for 8 weeks, and it has been good since. That was over 3 years ago and no problems since.
Casts are sort of the last resort, and it is awkward wearing them when they extend under your toes, but is better than surgery. My wife even painted my toenails to match my casts.
