I would like to have a small conversation (no email questionnaire) with members who left our association. Objective: find out the REAL reason why they left instead of 'too busy'. Any other suggestions about how to set up this type of conversations are also welcome. Thanks in advance.

I think the most honest and genuine answers you would get if you actually take the time and phone each person up and ask them for their honest opinion in a phone conversation, rather than send an email. Tell them you would like to improve things and would really appreciate some feedback as to why they left. Emails are more anonymous and many people won't get into detail or not even answer at all.
