Gangnam is a rich neighborhood in Seoul. The elite and "beautiful" people live there (maybe Korea's Beverly Hills?) The men and women there are fit and beautiful and healthy, and part of Psy's joke was that a regular looking guy like him would not live in Gangnam, so it was a funny reference. Hope this helps!
