What causes matter to change from one state to another?

The key of changing the state of matter are two: temperature and pressure. So, since the key of changing the state of matter are temperature and pressure, when there is enough temperature and pressure, there will be change in the state of matter.
For example, when you boil a water, you will put the water on the fire (creating high temperature) and close the container (creating enough pressure).
