What are possible ways or reasons that can cross-contaminate water supply lines.?

Back flow can be caused by the water main shut off and a faucet (tap) opened on a lower floor and one opened on an upper floor as air is sucked it can also draw in contaminated water from a sink or toilet that does not have a vacuum breaker ,air gap or air break or check valves although check valves are not considered back flow protection devices
