My wife has been a massage therapist for 18 years. She was certified in Guam, an American territory. She is an American citizen. After I left the Navy, we moved to Tennessee, less than a year ago. My question is As long as she puts it out there clearly that she is Guam-certified, can she legally have a massage therapy business out of our home?

Be very careful about that and check local laws and consult with a lawyer. Massage therapy is qute often used as a cover for and viewed by law enforcement as prostitution.
It wouldn't hurt if she gets training within your local area so she has a recent diploma hanging on the wall from a recognized school.
