... stage 3, does this mean something is wrong with my application?

Not likely, local government is usually slow to react and it is likely that it is just being processed. If you match the criteria to apply for a passport in your country and the photo is a match to what is required then it is likely that they are just delayed with others or that the person processing it has not updated the stage on the system.
I would not worry at all. If so, just give them a call and ask them, no doubt that will give a little nudge to getting it sorted.
