I need to write a letter to ask for a permission to take a semester off due to medical reason?

i am an international student and i have 2 year student visa. could you help me to write the letter? i dont know how to ask for permission.

just ask them to please approve a student visa  so you can attend school and if it is a different reason then include it as the reasons why you would like to go to school being a international student make sure you do the letter as to the reason why you want a permission letter start it out as could i get permission to attend school
