I'm the boxes where it tells you to write the type of ID your sending with your license (which is my birth certificate and my p45) I put my national insurance number next to my birth certificate (as the serial number) and I put my birth certificate serial number next to my p45, but I put a little curved double ended arrow next to the serial numbers to say that they need to be swapped, will they accept it or send me another form free of charge ?
