Have you talked to your doctors abt this.....ask him to send you to nutritionist who can set up a diet / exercise routine for you and keep you on track.
U need 2 involve in exercising ur body either jogging,press-up or other little ones base on ur capacity.Taking of fruits is very necessary and eat of energy foods such beans,fish,egg,rice,plantain, veg. etc in short balanced diet.
Gradually increase amount of fresh veggies in your ration - eat it on breakfast, on supper istead of bread, butter, sweets. Drink more water and go jogging
have you been to physical therapy - if not maybe this is also something to talk to your doctor abt.
