Puncture wounds are dangerous. Presumably a wound like this wouldn't bleed much. Immediate first aid would be to flush it copiously with hydrogen peroxide and then to cover it with a dry sterile dressing, usually 2 4x4"s and kling wrap or tape. Flush the wound with saline every 8 hours.
Watch closely for signs of infection: redness, abnormal tenderness, and pus. If it develops pus, the patient needs professional medical care.
