This is a bit of a weird question to ask, isn´t it? But Since I was a child I haven´t eaten very much. I´d get forced to eat and never do it by myself. The reason is that I don´t know when I am hungry, I don´t know how it feels.
I am very thin because of this.I know how it feels to be thirsty though. I remember my mother saying that it aches in your stomach and you feel a bit like you want to puke. My dad said differently though.
My stepmom said something else as well. How does it feel to be hungry?

The sensation of hunger is different for everyone. It could be a burning sensation that's right below your sternum, or a grinding sensation in your lower stomach. Some people get a stomach ache, some people start burping, some people, like you, don't have any sensation at all!
You should never just "eat when you're hungry." To be a healthy individual you have to eat a normal amount every day.
To combat your unique situation you should keep a diary and a strict schedule.
For instance:
1. 6 am: wake up, brush teeth, take shower, comb hair (activities of daily living)
2. 6:30 am: eat breakfast (have your meals planned out beforehand, whether it's cereal and toast, or eggs and bacon, just make sure it's something you look forward to or you'll skip breakfast)
Have this for every day. Some things you can probably copy and paste (get up at 6 am, wash face, etc.) but you can change what you eat for which meals. Instead of cereal for breakfast, try a sandwich if you prefer that.
Also, make sure to give yourself plenty of snacks throughout the day. If you're in school, put some in your backpack. Eat one or two before lunch and one maybe just after you get home.
Right now, you're at risk for inadequate nutrition because of a deficient intake of food and deficient knowledge. You should not be wondering "what does it feel like to be hungry," you should be wondering "what am I going to have for breakfast/lunch/dinner/snack?"
Do not only eat when you're "feeling hungry." Eat every day around the same time. I can't stress this enough. As it stands, if you still aren't eating enough, you might end up in a hospital with a tube down your throat. I could say I don't mean to scare you, but with something like this it's better to be honest and tell you what you can expect if you can't (or won't) eat when you should be eating (breakfast, lunch, dinner).
