This problem is common especially for small dogs, my chihuahua has the same issue.
Some things you can do are:
1. Don't always have your dog near you, if your dog is always by your side it will only show them they need to always be with you.
2.Put your dog outside while you are inside. Do it gradually, increasing time apart every time. When your dog comes back inside only contact them when they calm down.
3.Find out the anxiety triggers and practice them. Put on shoes and don't go out, put on your coat and watch tv, pick up car keys and walk around with them. This will get your dog used to these event without the anxiety of you leaving forever.
4.When your dog is calm with these situations that used to bother him, practice going out then coming back in. Maybe start with going out the door for a minute, then 20, then half an hour, and so on. Build up your dogs endurance.
