If you have an incorrect birth date in your passport you'll need to print out a copy of Form DS-5504 -- on one-sided pages -- and completely and legibly fill it out using blue or black ink.
In regards to social security:
Any individual, either U.S.-born or foreign-born, who requests a change or correction to the DOB on the Numident record, must submit evidence of age and evidence of identity to support the change, unless the correction is for an obvious keying error.
