Employer required by law to fill out DSS request for employee work history?

Local Department of Social Services provided a form to an employer to fill out an employee's work history. Time employed, hours worked, salary, etc. The Employer never returned the form to DSS. Are they required by any state or local law to do so? (New York State, but not NYC)
