Card placed out of position in dummy, consealed behind different suit, and created a revoke.

According to the laws of duplicate bridge, the concealed card is considered to have been visible for the sake of rulings (law 14b4 - the laws are free as PDF). Therefore if dummy already failed to follow suit, technically there has been a revoke.
