I offered a 6 month hold she declined. she is just not happy with us and has been a customer for 4 years

Ask her what she is unhappy about! Did the pests not go away? Did they return right away? Was your customer service bad? Did you not wipe your feet or put shoe covers on before entering her home. Were you late for an appt and made her wait and didnt call ahead?
Tell her it is your desire to improve your company and your employees and you can't do that with knowing the direction to take and her input would be greatly appreciated.
Once you know you might be able to go from there maybe give her one free service if it is an actual problem that you can fix. It might be she has a family member or friend in the business and wants to use them. That's something you can't fix. So you leave a business card and say if she ever changes her mind to please call and thank her for being a long time customer. Dont burn bridges because word of mouth advertising can help and ruin so remember customer service.
