If he's never called in sick, as opposed to scheduling sick days for a family member needing medical attention, as opposed to the employee him/herself being sick, does that still count as an excellent attendance record on an evaluation?

It would depend on the company you work and their written guidelines I know from company to the company from being sick to them is like interfering with the attendance record unless you had talked to the company. Dry note taking you off work for a certain amount of time if your company looks down on that type of attendance they could use that in your evaluation of your pay increase unless they said don't worry about it.
But what if an employee requested sick leave just one time that year (asked one week in advance if he can use two sick days)?
