Can I chose who to donate my organs to?

If you are a living donor, you can choose who to donate your organs too. Most likely you would be donating a kidney, and you can donate a kidney to someone in need of your choice. But if you wanted to donate your organs when you die, how would you know when you will die and who will be in need of an organ at that time? If you are healthy, would you expect to outlive those in need of organ transplants? This is an important aspect of the question to include.
